The $499 SAP evaluation package includes the initial evaluation, written recommendations and referrals, the follow-up evaluation after documented compliance, applicable SAP reporting, and the follow-up testing plan.

Budget beyond the evaluation fee
A lower evaluation price does not eliminate the requirements that follow an assessment. Recommendations are individualized. Ask education and treatment providers for their fees before enrolling, and confirm who pays for testing with your employer.
“One price” refers to the SAP evaluation package. It does not mean every service in the entire DOT return-to-duty process is included.
